Targeted Homeless Case Management

Targeted Homeless Case Management (THCM) is a Maine Care-funded service offered at our shelter in order to provide assistance to our guests in attaining necessary community services. Some examples of the services coordinated by our Case Managers are Maine Care, TANF, Primary Medical Care, Vocational Rehabilitation, Mental Health Services, Children’s Services, Adult Education, Substance Abuse Treatment and Support, Transportation Services, Youth Support, as well as many others. THCM services have been delivered to nearly 300 guests in 2016 by two 35-hour-a-week and two 20-hour-a-week Case managers. Targeted Case Management Services were provided to 24 clients after they left the shelter in 2017.

Section 13 Targeted Homeless Case Management provides case management services to current shelter guests in order to support our guests with accessing needed services including, Medical insurance, Primary care providers, mental health support, Transportation, Disability benefits, etc.

  • Shelter Intake
  • Referral to primary care providers
  • Referral to mental health services
  • Assistance with applying for benefits, Assistance applying for Maine Care and food stamps
  • Establishing a Plan of Care
  • Ensuring services are being provided by monitoring outcomes
